please state preference for times/ days etc. and leave a contact number if appropriate.  Please specify if you are requesting an appointment for smoking therapy as this does not require an Initial Consultation and a longer appointment will be needed.  Emails will be answered within 24hours wherever possible.

Although every effort will be made to meet requests for times/ dates the practice is generally well booked and it may be necessary to offer an alterative.
